Request for Use of Sick Bank Days
From the Contract: ARTICLE 27
SICK LEAVE BANK
27.1 A voluntary sick leave bank shall be maintained for use by eligible members who have exhausted their own sick leave accumulation through an illness. Sick leave bank days can only be used for the eligible member’s own illness, injury, or disability.
27.2 The Sick Leave Bank shall be maintained by the Association and administered by a Sick Leave Bank Committee composed of three (3) Association members designated by the Association, and two (2) members on behalf of the school district, designated by the Superintendent. The Sick Leave Bank Committee shall determine the eligibility for the use of the bank and the amount of leave to be granted. The following criteria shall be used by the Sick Leave Bank Committee in administering the bank and in determining eligibility and amount of leave:
a. Adequate medical documentation of the member’s own illness, injury, or disability. b. Propriety of use of sick leave by the member.
c. In cases related to a prolonged illness, sick bank days shall be awarded in increments.
27.3 Operation of the Sick Leave Bank and withdrawals there from shall be carried out by the Sick Leave Bank Committee, which shall furnish written records of deposits and withdrawals to the Superintendent together with a copy of the administrative regulations established and any amendments thereto.
27.4 Each eligible member who wishes to participate shall submit one (1) sick day of their personal accumulation to the Sick Leave Bank. No sick days assigned to the Bank may be recovered by the Assignor.

27.9 Members shall be eligible to donate days to the Sick Leave Bank whenever the Association opens a donation period.
